Optin subscription - empty list

Subscription works, but the list shown is empty:

Only Public lists are shown on the public subscription page. Are the lists that are not appearing public?

Yes the list is public. The E-Mail Template shows the list correctly.

Is this supposed to be the "Unsubscribe" link? Public URLs have the /subscription prefix which is missing in the example you have shared above.

Yes /subscription/form shows the public list.

The optin confirm page shows only this:
